| Requirements and Responsibilities For Discharge Planner |
| Coordinates patient discharge planning activities to ensure safe, timely, and effective transitions from healthcare facilities to home or other levels of care. Assesses patient medical, psychosocial, and post-discharge care needs in collaboration with physicians, nurses, social workers, and interdisciplinary care teams. Develops individualized discharge plans that address treatment continuity, rehabilitation services, home health support, durable medical equipment, and community resource needs. Communicates with patients, families, insurance providers, and external care facilities to facilitate discharge arrangements and resolve barriers to care transitions. Monitors discharge progress, documents care coordination activities, and ensures compliance with healthcare regulations, patient privacy standards, and organizational policies. May require a bachelor's degree. Typically reports to a manager. Occasionally directed in several aspects of the work. Gaining exposure to some of the complex tasks within the job function. Typically requires 2-4 years of related experience. |
